"Come to College Day" Information Sheet & Registration Form

1. Please fax or e-mail response no later than Monday, April 14th to FAX: (510) 642-0507

2. All Groups: Youth group members will receive game tickets upon entry into Edmond's Diamond (enter through the main doors) as a group only. Your entire group must enter together for free admittance. Please note the number of participants in your group that may require wheelchair seating. In addition, in order to receive your tickets, the amount listed on your roster must be as accurate as possible when arriving.

3. Group Leaders: Due to NCAA Rules, complete the following form or sign up online by providing your contact information to reserve a spot for your group. We want to make sure that your group will receive free tickets and take advantage of this special day. Reserve your spot by signing up now!

4. Once determined, please submit the registration form with the correct number of participants-up to one adult chaperone per 8 youth participants in advance. Cal Athletics will confirm your request and provide more information to you via mail or email. The names of youth and chaperones who will attend must be submitted no later than Monday, April 14, 2008.

5. All Youth 12th grade and under are invited to participate in this Come to College Day promotion.
